Reschedule Request Form

This form is used to request a one-time reschedule for an Executive Hotline or COO Strategy Intensive session. Please complete all fields accurately and provide three (3) preferred new session dates and times, listed in order of preference, selected from the currently posted availability. Submission of this form does not guarantee approval, and all reschedule requests are subject to availability.


If your reschedule request is approved, you will receive an email with a payment link for the applicable administrative fee. The new session date and time will not be confirmed until the administrative fee has been received. If payment is not received by the due date indicated in the payment link, the reschedule request will be cancelled, the original session will be forfeited, and no refund will be issued. You will receive a confirmation email once your rescheduled session has been finalized.
